The castle part is the free half. Each of these stops in the courtyards or passes the walls for twenty to thirty minutes, and none of them buys the museum ticket. That is not a flaw; it is the shape of the product, and knowing it lets you plan the rest.

You get a held Last Supper reservation, a guide, and a walk that takes in the castle's tower and courtyards. Two of these also carry a ticket for the Duomo, and one is private, for up to four.
What you do not get is the inside of the castle. Each listing names the castle admission in its exclusions or stops without a ticket. If you want the Pietà Rondanini, add the €10 ticket for the afternoon, or book a guided castle tour on another day.
Book as early as you can, because the refectory slots, not the price, are what run out. Give every traveller's full name exactly as on the ID you will carry. Leave bags at your hotel: one of these says bags of any size are not allowed in the Last Supper museum.
